  • Bleeding when passing stools (blood in the poo poo). Bleeding from the back passage. The mucosa or membrane that covers the haemorrhoids is quite delicate and if it's damaged there may be bleeding. This is usually seen as small amounts of bright red blood on the toilet paper or surface of the motions. Occasionally, they cause severe bleeding.

  • An itchy feeling around the anus or back passage.

They're usually soft and fleshy but may become hard if thrombosis occurs (when a clot forms in the blood vessel, blocking it). They may only appear after straining on the toilet.

Blood vessels are a normal part of the anal canal (the last section of the bowel) and, along with other tissues in this area, help to keep the anus closed. However, the blood vessels can become swollen, often as a result of straining to pass a stool.

Hemroids/Piles Symptoms

Haemorrhoids piles are usually mild and, in most cases, the symptoms will go away naturally within a few days. Most people may not even realise they have haemorrhoids as they do not experience any symptoms.

The symptoms of piles haemorrhoids include:
  • Bleeding after you have passed a stool (this will be bright red blood)
  • Discomfort around your anus, such as feeling sore or it being visibly red
  • Discharge of mucus present after passing a stool
  • Excessive straining when passing a stool
  • Feeling like your bowels are still full and need emptying
  • Itchiness around your anus (the opening at the end of the digestive system where solid waste leaves the body)
  • Pain while passing a stool

Types of Piles Hemorrhoids
Haemorrhoids can be:
  • Internal piles/hemorrhoids
  • External piles/hemorrhoids
Hemorrhoids vary in size and can occur internally (inside the anus) or externally (outside the anus).

Internal piles/haemorrhoids
Internal piles haemorrhoids are classified using a grading system from one to four.

Grade 1 and Grade 2 Internal haemorrhoids Grade 1 piles/haemorrhoids are small swellings often inside the lining of your anus. They cannot be seen and are very common. In some cases, they will enlarge to grade two.

Grade 2 piles/haemorrhoids are larger in size but are still within your anus. Sometimes they become pushed out when you pass a stool, but will return inside immediately after.

Grade 3 piles/haemorrhoids appear outside your anus. You may be able to feel one or more small lumps hanging out. They are also referred to as prolapsed haemorrhoids. You will usually be able to push them back inside using your finger.

Grade 4 piles/haemorrhoids can become quite large and remain outside your anus permanently. They cannot be pushed back inside and will need to be treated by your GP.

Internal piles/haemorrhoids develop 2-4cm above the opening of the anus.

External piles haemorrhoids
External piles/haemorrhoids, also known as perianal haematoma, are small lumps that develop on the outside edge of the anus.

Grade 3 and Grade 4 External haemorrhoids They can become increasingly itchy.

If a blood clot forms inside them (thrombosed external haemorrhoid), it can cause a lot of pain, requiring immediate treatment.

Estimates for how common piles/haemorrhoids are vary, as often any symptoms to do with the anus, such as bleeding, are thought to be haemorrhoids, when it could be another condition.

In the most countrry, it is estimated that between 4% and 25% of the population is affected by haemorrhoids.

Anyone at any age can be affected by haemorrhoids. However, they are usually more common during pregnancy and in people between 45 and 65 years of age.

  • Consider referral to secondary care if there are severe symptoms or complications.

Increase Fibre or Fluid Intake
  • Increasing fibre and fluid intake is the most important aspect of conservative management.
  • It may take 6 weeks before improvement is apparent.
  • The aim is to:
    • Soften the stools.
    • Reduce the need for straining.
  • This can be achieved by:
    • A high fibre diet - rich in fruit and vegetables, whole grain cereals and adequate fluid intake (2L or 8 glasses per day of non-caffeinated fluids).

    • Fibre supplements or bulk providing laxatives (E.g. methylcellulose, sterculia, ispaghula husk) accompanied by adequate fluid intake.

    • A systematic review found that fibre based laxatives had a consistent beneficial effect on symptom relief and bleeding.

Constipation is when someone goes to the toilet less often than usual, or when it's difficult to pass a stool. It's a change from a person's usual pattern of bowel opening.
